Registering for a dog obedience class

To register for a class, you must fill out an application to give us information about you, your dog, your dog's veterinarian, your dog's most recent vaccinations for rabies, DHLP (distemper, hepatitis, leptospirosis, parainfluenza), and parvovirus, your dog-training experience, and, finally, how you heard of our classes.  

After you fill out the application form, we will give you a release form that you must sign.  

Your application will be reviewed, and our Club Secretary will contact you to arrange for payment for the training, as we do not accept payments online.  

At the time that you make your payment, you will also be asked to sign the release form and to provide us with either a receipt or health certificate from your veterinarian as proof of your dog's rabies, DHLP, and parvovirus vaccinations.  Tags will not be accepted as proof of vaccination.
